you need to connect the double satellite feeds to your box - i'm assuming
you have a dish fitted here.
then you phone sky and ask them to pair your viewing card to your new box.

if you dont have a dish with 2 feeds then you will have to pay to have one
fitted - unless you are up to doing it yourself.

Not from sky either - they will not install equipment they do not supply.

If you already have Sky, you can buy a Sky plus box and connect it up
with just LNB1 if you like. You will then have to phone Sky and get
your account registered as a Sky plus account (phone line not
needed).
You will not be able to watch one Sky channel and record another as
you have only one LNB connection.
You will not need to pay any more per month so long as you are a Sky
subscriber already.

If you want to use the full capability of the Sky Plus, you will need
a twin output LNB on the dish and two downleads.

If you dont have Sky already, you will have to sign up as a new
subscriber. In this case you have two options, get your own box and
have it installed by anyone who is capable and then Call Sky and ask
them to set up an account, or go directly to Sky or a registered
Independent Sky Dealer as a new customer and get them to do the whole
thing.
The installation will generally be much neater and more complete if
you go to an Independent Sky Dealer as they usually have a much wider
knowlege of aerial system and equipment installation.

First you have to have a dish installed. It also needs to have dual
cables to make the twin tuners work, so at least a dual LNB on the
dish is needed. A phone line has to be attached aswell and a working
viewing card has to be matched to the box.

I upgraded my sky to 18 mths ago. I fitted a new quad LNB (the lump
on the sat dish) myself, ran the second cable (needed to watch/record
from sep chanels) and connected it up. A simple DIY job with no
specialist tools req'd as I didn't disturb the alignment of my dish.
I've never had my box connected to the tel line since the initial
installation 6mths before.

Sky's only involvement was to pair the card to the box and activate sky
+ on my account. As I have the sports and movie chanels there was no
charge. The total cost of the upgrade was about £120 with a new box,
cable and LNB all from ebay. I ran some spare cable and use the old
box as a freesat box in my bedroom from one of the unused outputs on
the quad LNB.

Anything so long as you are a subscriber of some sort.
