Default Dual tuner over one coaxial cable

Is it possible to receive use both tuners on a dual tuner DVR using one
coaxial cable? I know about multiplexers, which allow for OTA and satellite
to share a cable, but they are different frequencies.

Do I always need to run an extra cable in an existing location that only
has one?

Not sure what you're talking abpout, but I only have one coax from my
antenna to my 4 tuner dvr (PC). Eqach tuner requires it's own connection,
so I use a 1 to 4 splitter. I gues if I'd designed a consumer device, I'd
build the splitter in the box.

Sorry, should have been more clear... I am talking about for a DirecTV
application. Know anything about that?

What you are looking for is a Stacker, such as:

I've never actually used one, so I can't comment on the reliability.
Don't forget, you'll need a Destacker on the other end, which the
description mentions.

With DirecTV, you need 2 cables unless you install special equipment
(a stacker). With one cable, you can only record one channel at a
time. Moreover, I understand that stackers don't work with HD DirecTV.

Directv has been working on a single-cable solution, which they call a
Frequency Translation Module, however it is not yet available. Some of
the newer receivers (HR20, for example) are equipped to work with this.

It is possible.

I have Dishnetwork and two receivers.
my 501 DRV receiver is on it's own cable,
My 622VIP is fend by a second cable that is also feeding my OTA down from
the roof using a diplexer that cam with my RadioShack roof top antenna.
in the house at the other end of the cable, the diplexer splits the OTA from
the SAT feed, and from there the sat feed is split by a Dishnetwork
"Separator"to feed both tuners in the 622
