Is it possible to order sky multiroom but provide your own 2nd digi box and
therefore not have to pay £99?

Yep - I did this.

A mirror of my main subscription, but without the charge for the 2nd
digibox. I do pay the additional monthly fee to receive the mirror
subscription.

I assume the montly fee is £10 ? And because not a "subsidised" box,
no phone connection?

Mirror sub (a.k.a. multiroom) = permanent and obligatory phone
connection for both boxes + £10/month fee. No ifs or buts.

It is £10 per month, but the box is connected to the telephone line (I
don't know if this somehow prevents relocation of the 2nd box & card
to a friend/family member for less than they would have to otherwise
pay for the subscription).

Did you have to alter your dish (eg add a second LNB), or did it
mirror and play all channels regardless of polorisation?

About a year ago I upgraded to Sky+ which resulted in a new LNB with 4
feeds, two of which were used by the Sky+ box. The original digibox
was kept, and this is what I used for my multiroom installation which
used the 3rd feed from the LNB. If I hadn't gone for Sky+ then the
LNB would need to have been upgraded as part of the multiroom
installation.
