I think that is covered by the mention of cable and splitters. It goes
without saying that any sort of bad connection can have all sorts of
side-effects.

Presumably it just wasn't made correctly in the first place. No sign

No, its perfectly OK in that respect, the best way I could describe it is as
if the centre wire has been pulled back over time, as if there was tension
on it (which there isnt)
