Interestingly it looks like the braid has been wrapped in a way which makes
it appear like it was intended to be clamped, have they got a young,
unsupervised, apprentice, or could it be an elderly rigger :-) having a
senior moment?

Trying to avoid the suggestion of rank ignorance or incompetence,
Looks to me as though perhaps the outer clamp wasn't tightened up,
either at all or enough, and pulling on the cable has pulled the outer
down the inner.

That is a good suggestion. Some coax will do that. The actual offending
object will be brought to me tomorrow. I'll have a look.

Nah. It's been installed like that. The clamp looks tight. The
un-snipped inner also suggests a carefree, sloppy installation.

Looks like it wasn't tightened at all. Otherwise some of the braiding
would have been dragged out instead of being still neatly twisted. I
think the installer simply didn't finish the job properly.

It obviously won't work like that! It would be interesting to know
whether it's ever worked - so that something must have happened to it
subsequently - or whether it was "installed" like that.
